Our Story
Hope Pathway Foundation was established to address the critical gap in services for youth aging out of the foster care system. Many young adults face housing instability, lack of support, and limited access to resources as they transition into adulthood.
Our organization provides housing-focused services, mentorship, and life skills development to help youth build independence and long-term stability. Our approach is rooted in trauma-informed care, cultural responsiveness, and a strengths-based perspective.

About the Founder & CEO
Jazzmin H. Hughes MS
Jazzmin H. Hughes is the Founder and Chief Executive Officer of Hope Pathways Foundation, a nonprofit organization dedicated to empowering foster youth, transitional-aged youth, and vulnerable individuals through mentorship, advocacy, supportive services, and pathways to long-term success.
Jazzmin holds a Bachelor of Arts degree in History and Education, a Master of Science degree in Higher Education Student Administration, and a minor in Adult Education. She is currently pursuing a Master of Social Work degree and working toward licensure as a Licensed Clinical Social Worker (LCSW), further expanding her expertise in trauma-informed care, mental health, and therapeutic practice.
With more than a decade of leadership experience in the nonprofit and social services sectors, Jazzmin has overseen programs serving youth, families, veterans, and individuals experiencing homelessness. Her professional background includes program management, staff supervision, housing and supportive services administration, family reunification services, juvenile justice programs, migrant services, and community partnership development. Throughout her career, she has successfully led multidisciplinary teams, managed complex programs, and advocated for underserved populations.
As an emerging social work practitioner, Jazzmin is committed to a strengths-based, person-centered, and trauma-informed approach that promotes healing, resilience, and self-determination. She is passionate about helping individuals navigate life’s challenges while fostering personal growth, emotional wellness, and long-term stability.
Through Hope Pathways Foundation, Jazzmin is working to create meaningful opportunities for youth and young adults by expanding access to mentorship, education, housing support, life skills development, and community resources. Her vision is to build a lasting organization that empowers individuals to achieve their full potential and create brighter futures for themselves and their communities.
Board members
Daquan Hubbard, President
Daquan Hubbard serves as the President of Hope Pathway Foundation. He is an Air Force veteran with a strong commitment to leadership, service, and community support. His military background has strengthened his ability to lead with structure, accountability, and purpose.
Daquan is dedicated to supporting the organization’s mission by helping guide strategic decisions and ensuring the foundation continues to provide meaningful support and opportunities for transitional-aged youth.
Demarcus Reed, Vice President
Demarcus Reed serves as the Vice President of Hope Pathway Foundation. He is a United States Marine Corps veteran and a former foster youth, bringing both lived experience and leadership to his role. As a Purple Heart recipient, he demonstrates resilience, strength, and a deep commitment to service.
Demarcus Reed holds an Associate of Arts in Criminal Justice, a Bachelor of Arts in Human Services, and a Master of Social Work from the University of Southern California. He is currently pursuing a Doctor of Philosophy (PhD) in Human Services at Walden University.
Demarcus is passionate about supporting youth who have experienced similar challenges and works to ensure the organization remains focused on creating real pathways to stability and independence.
Dennis Dickens, Treasurer
Dennis Dickinson serves as the Treasurer of Hope Pathway Foundation. He brings experience in financial oversight and organizational support, with a focus on accountability and responsible resource management.
Dennis is committed to ensuring the organization maintains strong financial practices and sustainability, helping to support the foundation’s mission and long-term growth.
Shavon Morgan, Secretary
Shavon Morgan serves as the Secretary of Hope Pathway Foundation. She is a dedicated human service professional with over 20 years of experience supporting individuals from diverse age groups, cultural backgrounds, and life circumstances. She brings strong organizational skills and attention to detail, supporting the foundation through documentation, coordination, and communication.
Shavon plays an important role in maintaining structure within the organization and ensuring that processes run smoothly as the foundation continues to grow.
Sandra Hughes, Board Member at Large
Sandra Hughes serves as a Board Member at Large for Hope Pathway Foundation. She brings more than 15 years of behavioral health experience working with underserved and underprivileged populations, including youth, individuals experiencing mental health challenges, and vulnerable community members in need of support and advocacy.
